Registering with Randwijck GP practice

We are happy to welcome new patients. You can register with us if you do not yet have a family doctor in Amstelveen and lives in postcode area 1181, or if you you are a student and live at Uilenstede. Do you fall within this target group? If so, you are welcome to register with us.

Practical information about your registration

• On your first visit, we ask you to bring valid identification (passport or ID card) and your health insurance details.
• By registering with us, you give us permission to deregister you from your previous GP. We also request that you inform your current GP yourself that you are switching.
• Please note that completing and submitting the registration form does not automatically mean that you are immediately registered. Once we have received your form, we will contact you as soon as possible to let you know whether registration is possible.
