Registering new patients

We are happy to welcome new patients. You can register with us if you do not yet have a family doctor in Amstelveen and lives in postcode area 1181, or if you you are a student and live at Uilenstede.

Do you fall within this target group? Then you are welcome to register with us. This is how you register:

1. Print out the appropriate registration form (child or adult, Dutch or English) and fill it out at home.
2. Hand in the form in person Monday to Thursday between 08:00 and 11:30.
3. For each person individually, you should enter the insurance card and a valid identity document (passport or identity card) with you.

Your registration will be processed within five working days.
