Register new patient

The practice is open to new patients who do not yet have a GP in Amstelveen and reside in postcode area 1181. The registration form can be collected at the practice from the assistants on Monday to Friday between 08:00 - 12:00. The assistant will also ask for your insurance details at that time.

GPs then invite new patients for an introductory interview. In this conversation, you can bring important matters about yourself to the GP's attention, such as illnesses, allergies to medicines or major life events. Of course, you can also ask your own questions about what is going on in the practice.
