As a practice supporter GP somatics (POH-S), you provide information and advice to chronically ill patients. You support them in breaking unhealthy habits in the field of exercise, nutrition and medicine use. Are you eager to motivate patients to live healthier and more active lives?

Practice counsellor somatic
As a POH-S, you will assist patients with chronic conditions such as diabetes, COPD, asthma, and cardiovascular disease. You will conduct consultations independently and advise patients on lifestyle, exercise, medication use, and nutrition. Your duties will include:
You work closely with the GPs in the practice. You will discuss the care plans you draw up with the GP, and in complex cases you will ask the GP for help.
Huisartsenpraktijk Randwijck is part of Medisch Centrum Randwijck, which also houses a pharmacy, physiotherapist, social worker and psychologist. The patient population of 10,000 is diverse and consists largely of expats and students from the Uilenstede district.
The team at Randwijck General Practice consists of eight general practitioners, three practice assistants specialising in somatic and elderly care, two mental health practice assistants, six doctor's assistants and a practice manager. Practice manager Susan Meijer describes the working atmosphere as a close-knit second family. “People listen to you and look at your personal qualities, which are then responded to. You can always turn to anyone. Everyone thinks along with you, is happy to go the extra mile, and nothing is too crazy,” says Susan.
Despite the high workload, the practice ensures that there is also room for relaxation. There is a lot of laughter and working days end with a ‘What went well?’ moment. Together with colleagues, you look back on the events of the day that you are proud of or that were special, so that you go home feeling satisfied.
You have completed the POH-S training programme. You also have an affinity with elderly patients and are motivated to follow the Elderly Care Practice Support Worker (POH-O) training programme in the future. In addition, you bring the following to the table:
Randwijck General Practice is looking for a POH-S for 24–36 hours per week. Your working days will be determined in consultation. You will receive a one-year contract, with the prospect of a permanent position.
