POH-S at general practice Randwijck

As a practice supporter GP somatics (POH-S), you provide information and advice to chronically ill patients. You support them in breaking unhealthy habits in the field of exercise, nutrition and medicine use. Are you eager to motivate patients to live healthier and more active lives?

What will you do?

As a POH-S, you supervise patients with chronic syndromes, such as diabetes, COPD, asthma, and cardiovascular diseases. You carry out independent consultations, giving patients advice on lifestyle, exercise, medication use and nutrition. Your duties consist of:

  • Intake and anamnesis
  • Counselling and coaching patients on how to cope with their clinical picture, modify their lifestyle and promote their health.
  • Providing information and advice to patients on medical matters
  • Collaborates with professionals in care and welfare and refers patients to them if necessary
  • Maintaining care records (own consultation)

You work closely with the GPs in the practice. You will discuss the care plans you draw up with the GP, and in complex cases you will ask the GP for help.

About the practice

Huisartsenpraktijk Randwijck is part of Medisch Centrum Randwijck, which also houses a pharmacy, physiotherapist, social worker and psychologist. The patient population of 10,000 is diverse and consists largely of expats and students from the Uilenstede district.

The team at Huisartsenpraktijk Randwijck consists of eight GPs, three practice support staff for somatic and elderly care, two practice support staff for mental health, six doctors' assistants and a practice manager. Practice manager Susan Meijer describes the working atmosphere as a close-knit second family. "You are listened to and your personal qualities are taken into account. You can always contact anyone. Everyone thinks with you, likes to go the extra mile, and nothing is too much trouble," Susan says. Despite the high workload, the practice ensures that there is also room for relaxation. There is a lot of laughter and working days are rounded off with a 'What went well?" moment. Together with colleagues, you look back on the day's events that you are proud of or that were special, so that you go home feeling satisfied.

You have the

You have completed the training to become a POH-S, or you have completed a medical training and are willing to follow the training to become a POH-S. In addition, you have an affinity with elderly patients and are motivated to train as a Practice Support Person for Elderly Care (POH-O) in the future. In addition, you bring:

  • If your work requires collaboration, seek it out within the team.
  • Affinity with lifestyle coaching and knowledge of common chronic conditions such as cardiovascular disease, type 2 diabetes mellitus and asthma/COPD
  • With your enthusiasm, you know how to motivate people to change their behaviour.
  • You are approachable and make people feel comfortable with you.
  • You work independently, take responsibility and set the right priorities.

GP practice Randwijck is looking for a POH-S for 16 hours a week. If interested in more hours, this is also possible at the practice. Your working days will be determined in consultation. You will receive a one-year contract, with the prospect of a permanent appointment. Salary and terms of employment are in accordance with the collective agreement for GP care, depending on your experience. You can count on:

  • A gross salary of €3,594 to €4,714 based on a 38-hour working week (scale 7).
  • A year-end bonus equivalent to a 13th month.
  • 190 holiday hours per year based on a 38-hour working week.
  • Support from the regional organisation (Amstelland Care) through various trainings and intervisions.
  • Opportunities to specialise in a particular disease area or combine this position with the role of POH elderly.
