In people with chronic diseases such as diabetes, high blood pressure, renal dysfunction, asthma and COPD and cardiovascular disease, periodic monitoring is necessary. The GP somatic practice support person (POH-S) does the supervision of these chronic conditions within a regular consultation hour. The results of the check-ups are discussed with your GP and, if necessary, the GP adjusts the treatment. When the POH-S invites you for an appointment, it is always at the request of your GP.
The POH-S has undergone training that enables her to independently draw up a treatment plan for the above-mentioned patients. As there are continuous developments in her field, she attends many refresher courses every year. Where necessary, the POH-S consults with the GP.
There are no costs associated with the care provided by the POH-S. The costs are reimbursed by the basic insurance. Your excess will not be charged, as the care falls under GP care.
